Preparing for your first visit
Welcome to our practice! Please print and complete the Patient Registration & Medical History form to the right and bring it to your visit. You do not need to complete this form if you are registered on our patient portal and have completed the exam interview. Please also bring, if applicable, any current eyeglasses and contact lens boxes or prescription information to your exam.

Please verify with your insurance our participation with your plan.
- All copayments and examination fees are due at the time of service. We accept cash, check, Visa, MasterCard and Discover.
- Payment in full is required to place an order for eyeglasses or contact lenses.
- If necessary we will submit a claim to your medical insurance. Please understand that we do not know if your insurance plan will approve particular services. Co-insurance and deductibles are the responsibility of the patient or guarantor and payment is due within 30 days of receiving your statement.
Appointments are required to see the optometrists, however you do not need an appointment to see one of our opticians in our optical department. Please give us at least 24 hours notice if you need to cancel or reschedule your appointment.
